Eligibility Specialist Northland CARE

Description:

Provides day to day coordination and oversight for Northland CARE patients. Northland CARE is a primary care physician referral network for low-income, uninsured patients in Clay and Platte Counties. Assist with program implementation and evaluation; compliance with laws and regulations regarding confidentiality and privacy issues; maintenance of records and data storage: communications with other workgroups and community partners. All work is directed by NHCA Executive Director

Responsibilities

Function 75% of Time
Program Management: Responsible for screening patients for edibility for Northland CARE program, accepting and processing all Northland CARE patient referrals, making referrals to MetroCARE for specialty care and providing case management services for current patients. Follow all program policies and procedures, quality assurance procedures; and program evaluation activities. Work in collaboration with other staff.

Function 5% of Time
Public Relations and Presentations: Serve as clinical staff liaison to Clay and Platte health departments and other customers.

Function 20% of Time
Administration: Provide accurate & timely reports on community contacts and Northland CARE patient referrals, physician pledges and current patient status on a monthly data report.

Qualifications Physical Requirements:
Work Environment Normal office environment
Lifting Requirements 25 pounds or less
Travel Requirements Occasional local driving
Other Must follow prescribed policies and procedures
Drivers License Must have current driverýs license and driver coverage for personal vehicle and reliable transportation

Education and Experience Requirement:

Required:
Previous experience in clinical, social service and/or health care settings; must have working knowledge of clinical issues and community resources; must be computer proficient, data oriented and outcomes focused; positive community relationships essential.

Preferred:
Advanced education in public health, community health or social services preferred. Preferred characteristics include a responsible and enthusiastic person capable of quickly establishing respect and trust from others, with solid leadership and management skills. Should have exceptional interpersonal skills with an ability to relate well and communicate effectively across diverse ethnic, educational and socio-economic lines. Must be a flexible, non-judgmental leader, who possesses a highly professional demeanor. Must be able to function well under pressure, be a logical thinker with excellent problem-solving skills and be a strong advocate of teamwork with a dedication to quality improvement, maximizing productivity and effectiveness, and the promotion of an environment of learning, respect and collaboration.
